Daily charting and eMAR for group homes

Updated October 9, 2026

For DSPs, nurses and supervisors. Staff only see residents at their assigned houses.

Shift notes

  1. Start a shift note and pick the resident, service date, and Day or Overnight.
  2. Day shifts: score each support task (for example Independent, Prompted, Partial assist or Refused). Add a short comment for any task that isn't N/A.
  3. Overnight shifts: record a check and an observation for every hour.
  4. Write the narrative and record progress on ISP goals.
  5. If there was an emergency safety intervention, document it in the safety section.
  6. Sign and attest to finish. Unsigned notes stay drafts.

You can dictate where your browser supports it. An optional AI button can polish a narrative once per resident per day, using your AI calls. Signed notes export to PDF.

Med pass (eMAR)

Orders live on the resident's chart: dose, route, schedule, and routine or PRN.

  1. Select a scheduled dose.
  2. Choose a result: Given, Refused, Held, Vomited, Unavailable or Out of facility.
  3. Sign to save. Signed entries are locked.

CONQER enforces these rules:

  • The ±60-minute window. A dose outside the window needs a linked incident report, and an early dose also needs the prescriber's verbal OK.
  • PRN doses need a reason. CONQER then reminds staff to record whether the medication worked.
  • Corrections to a signed entry go through an incident report, so it's clear who changed what. No role is exempt.

Nurses can also keep a shift-change count book for controlled medications and import medication lists from a pharmacy spreadsheet (200 rows at a time).

Vitals

  • When a dose needs vitals first, record BP, pulse, temperature and weight on that med pass.
  • Nurses can record ongoing vitals for each resident and see recent readings at a glance.

Incident reports

  1. Start a new incident report and pick the resident and type, such as physical aggression, elopement, self-injury, medical emergency or medication error.
  2. Describe what happened. You can mark injuries on a body diagram and dictate the narrative.
  3. For a reportable event, choose its category. CONQER shows when the report is due (4 hours for urgent events, 72 hours for standard ones) and whether a phone call or state report is needed.
  4. Record guardian notification, add follow-up actions, and sign.

Deadlines are general guidance. Always follow your state's rules.
