Migrating from your current EHR

Updated October 9, 2026

Switching shouldn't mean retyping years of records. Migration is included with every plan, and our team is here for questions. You run the import.

What you can bring over

  • TherapCensus (CSV)Resident charts
  • TherapDemographics / IDF (CSV)The same charts, plus address, language and emergency contact
  • TherapeMAR / MAR historyPast med passes (given, not given, on hold, refused)
  • TherapGER incident reportsIncident reports
  • TherapISP program dataISP goals
  • TherapClinician reports (text or text-based PDF)Draft notes for clinician review
  • PointClickCareCensus, order sheet, MAR historyCharts, medication orders, past med passes
  • PointClickCareProgress notesDraft notes for clinician review
  • Any systemSpreadsheets of ISP goals, incidents or notesISP goals, incident reports, notes

You can export as CSV, Excel or JSON. Your column names don't need to match ours, because CONQER recognizes common Therap and PointClickCare names. Photos, custom forms and billing authorizations aren't imported.

Before you start

  • Add your houses first, since each resident needs a home house.
  • Leave Social Security numbers out of your export. If a column looks like one, CONQER warns you and skips it. Don't map it to another field. Social Security numbers are never imported.
  • Export dates as text (for example 01/15/2024).

Import steps

An organization admin runs the import. Staff roster imports also ask you to confirm your sign-in first.

  1. Add your file. CONQER reads it in your browser.
  2. Check the preview. Confirm the record type and that each column landed in the right field, and fix any that didn't.
  3. Review the dry run. Before anything is saved, CONQER shows how many records are ready and why any rows will be skipped.
  4. Import in this order: census, then medication orders, med history, notes, ISP goals and incidents.
  5. If a batch stops partway, choose Resume. Records already saved aren't duplicated.

Imported notes arrive unsigned, and a clinician reviews and signs them. Past med passes keep the original staff member's name.

Limits per file

  • Up to 20,000 rows and 6 MB
  • Up to 2,000 residents and 20,000 med-history entries
  • Up to 60 Therap note sessions or 40 PDF pages
  • Up to 300 staff per roster import

Split bigger exports into several files.

Staff come in as pending hires

Coming soon

Imported staff can't sign in, don't get an email, and don't use any seats. They're listed under Pending hires in Workforce, separate from your active roster and payroll. Activate the people you're keeping when you're ready. Activation uses a seat, and it stops when your seats are full.

How we help
